The book provides structured records for medications, meals, hydration, sleep, behavior, symptoms, safety concerns, appointments, and caregiver handoffs.
Caring for someone experiencing memory loss can require families to coordinate changing routines, medical information, safety concerns, appointments, and communication among multiple caregivers.
